PPO, Traditional Medicare, or Cash, call our office to schedule an appointment.
HMO insurance, make sure Lucita M. Cruz M.D. is your PCP or call your insurance and ask if you can be assigned to Dr. Cruz’s office. Once assigned to our office, call to schedule an appointment.
We do not accept new Medi-cal patients under the age of 65.

- Bring your ID and Insurance Card
- Bring all of your medications, in the bottles, if any.
- If applicable, please bring a way to pay your copayment, credit card or cash.
We will call 2 days prior to confirm your appointment. Please confirm your appointment or it may be canceled. Please call us if there are changes.
