Preventative services
We also offer preventative services such as Physicals, Well Woman exams and Pap Smears, Annual Wellness Visits for our Seniors, as well as comprehensive lab and imaging screenings.
Employment Disability
In addition, our office is able to handle competent and timely completion of Employment Development Department (EDD) Disability Forms for when you are temporarily unable to work due to illness or injury.

We also handle other forms such as In Home Support Services (IHSS), DMV Handicap Placards, Work Clearance Physical documentation, and letters from the provider.
Immigration Physicals
Lucita MD is a designated by the USCIS as a Civil Surgeon certified to complete form I-693, Report of Immigration Medical Examination and Vaccination Record.
