New Patients Info

How to become our patient

  1. We accept PPO, EPO, POS, Traditional Medicare, and HMO. We also accept patients without insurance who self pay. 
  2. For patients w/ PPO and Traditional medicare you can call our office and schedule immediately after we verify your insurance is active.
  3. For patients w/ HMO insurance you must be assigned to our office first by your health plan. If you are not assigned to our office you can select us as your PCP online at your Health Plan’s Patient portal or call the members services number on the back of your insurance card. If you need assistance, please feel free to call our office.If you are currently assigned to our office we can schedule you immediately after we verify your insurance is active.
  4. We also accept patients w/ Medi-cal if they are 65 years or older and are already assigned to an HMO.

New Patient Forms

Please fill out the following forms and submit one week in advance of your appointment:

    1. English Intake Form and No Show Policy
    2. Spanish Intake Form and No Show Policy

Know Before your appointment

  1. Come 10 to 15 minutes early
  2. Bring your ID and Insurance Card 
  3. Bring all of your medications, in the bottles, if any.
  4. If applicable, please bring a way to pay your copayment, credit card or cash.
  5. We will call/text 2 days prior to confirm your appointment. The appointment may be rescheduled if you arrive 10 minutes past your scheduled time. No shows may be subject to a $35.00 No Show Fee.

Know After your appointment

  1. Medications: Should be electronically sent to your pharmacy within 1 hour after your appointment.
  2. Lab and Imaging: We inform patients if their result has abnormal values. If everything is ok, results are filed without notice. Patients are welcome to call our office 5 days after the study is completed.
  3. Authorizations:Standard wait time for an authorization is 10 business days. If you do not receive notice of authorization status 10 business days from your appointment, please contact our office.
