How to become our patient

  1. Insurance we accept: We welcome patients with PPO, EPO, POS, Traditional Medicare, and HMO plans. We also accept self-pay patients without insurance.
  2. PPO & Traditional Medicare patients: Once we verify that your insurance is active, we can schedule your appointment right away. Simply give our office a call to get started.
  3. HMO patients: Your health plan must first assign you to our office as your Primary Care Provider (PCP). If you haven’t been assigned yet, you can request us through your Health Plan’s patient portal or by calling the Member Services number on the back of your insurance card. Need help? We’re happy to assist — just give us a call. Once you’re assigned and your insurance is verified as active, we can schedule you immediately.
  4. Medi-Cal patients: We accept Medi-Cal for patients who are 65 years or older and are already assigned to an HMO

To help us serve you better, please complete the appropriate form and submit it at least one week before your appointment:

Know Before your appointment

  1. Arrive early — Please plan to arrive 10 to 15 minutes before your scheduled time.
  2. Bring your ID and Insurance Card.
  3. Bring your medications in their original bottles, if applicable.
  4. Bring a form of payment for your copayment — we accept credit card or cash.
  5. Appointment reminders — We will call or text you 2 days before your appointment to confirm. Please note that arriving more than 10 minutes late may result in your appointment being rescheduled. No-shows are subject to a $35.00 No Show Fee.

Know After your appointment

  1. Medications — Prescriptions are electronically sent to your pharmacy within 1 hour of your appointment.
  2. Lab & Imaging Results — We will contact you if your results show any abnormal values. If everything looks normal, results are filed and no notification is sent. You are welcome to call our office 5 days after your study is completed if you’d like an update.
  3. Authorizations — Please allow up to 10 business days for authorization processing. If you haven’t received a status update within 10 business days of your appointment, don’t hesitate to contact our office.
